Brooklyn, NY (October X, 2022) – Today, Brooklyn Communities Collaborative (BCC) – a non-profit focused on strengthening health and wealth in Brooklyn – announces that it has been awarded two grants totaling $400,000 from the Altman Foundation, an organization that supports programs and institutions that enrich the quality of life in the city, and The New York Community Trust, the community foundation for New York City that along with its affiliates in Westchester and Long Island is dedicated to improving the lives of residents of New York City and its suburbs.

“We are thrilled to have continued support from the Altman Foundation and The New York Community Trust,” said Shari Suchoff, Executive Director of BCC. “This funding will support care coordination for Brooklyn residents as well as our community-driven workforce and health enterprise efforts over the next two years. We thank both organizations for understanding the importance of combatting the social determinants of health and improving quality of life in Brooklyn.”

BCC will use the $400,000 total award to promote and advance improvements in the economic, social, and community factors that impact the living conditions, and thus, the health of residents in Brooklyn. Over the next two years, both grants will be used to:

  • Support Maimonides Medical Center and One Brooklyn Health to reach their commitment to 10 percent minority and women-owned business enterprises (MWBE) procurement by adding 10 MWBEs as procurement partners over the next two years;
  • Enroll 90 individuals in training programs for high-need roles including property management and patient care technician to meet the workforce needs of Brooklyn;
  • Coordinate health and social services for 1,500 high-risk residents ineligible for Medicaid through expanded partnerships with local agencies.

“The New York Community Trust is proud to support Brooklyn Communities Collaborative, an organization that advances the quality of life and health in Brooklyn,” said Irfan Hasan, The Trust’s Deputy Vice President for Grants. “As a foundation focused on all five boroughs of our diverse City, BCC’s effort to provide the borough’s most high-need residents with healthcare and social empowerment will ensure all Brooklynites have access to equitable healthcare.”

“Harnessing the commitment and capacity of anchor institutions like medical centers and long-standing community organizations, BCC seeks to catalyze transformational change in Central Brooklyn,” said Rachael N. Pine, Senior Program Officer at the Altman Foundation. “We are pleased to support this effort to increase access to economic opportunity and to social care for high-need Brooklyn residents – individuals who daily struggle to overcome inequity and barriers to achieving their health goals.”

BCC previously received grants from both organizations in 2020. At the height of the pandemic, the Altman Foundation awarded a two-year grant of $150,000 in support of four initiatives aimed at promoting structural change necessary to addressing social determinants of health in Central Brooklyn. The New York Community Trust also granted BCC $150,000 in 2020 which helped strengthen relationships between healthcare providers and community organizations in ten central Brooklyn neighborhoods, which face some of the most significant barriers to health in the state.
